CEC/CEU forms are complimentary for 30 days following the Mania® and Live-Stream event . To receive your FREE current Mania® CEC/CEU form within the 30 day period, please go to the particular Mania® or Live-Stream event site here.

After this 30 day period, please use the form below to request a copy of your CEC/CEU form.  Please note that forms are available for download for previous attendees only.  There is a $35 fee for each download, please save your form in a secure place once received.  Along with a copy of your CEC/CEU form.
